OHIO DEPARTMENT OF PUBLIC SAFETY BUREAU OF MOTOR VEHICLES UNCLAIMED MOTOR VEHICLE AFFIDAVIT Section 4505.101 of the Ohio Revised Code (For Ohio Title issuance only. For use by Ohio licensed repair or storage facilities as verified by the Department of Taxation.) WARNING: Applicant giving false information in the execution of this affidavit is subject to prosecution under Section 2921.13 of the Ohio Revised Code (R.C.). Punishable by up to a one thousand dollar ($1,000) fine and / or up to six (6) months in jail. GUIDELINES Form BMV 4202 is to be completed by repair and storage facilities. This form can NOT be used for abandoned vehicles as described in R.C. 4513.60. This form cannot be used for: watercraft, trailers weighing less than 4,000 pounds, law enforced tows, property left on campgrounds. A valid vendor license is required for the duration of time the vehicle was received by your business through the filing of this form. Vendor licenses are verified through the Department of Taxation. If you need to purchase a vendor license, contact your County Auditor’s Office. A properly completed BMV 1173 Record Request form will be required with each mailed affidavit. Prepaid accounts will no longer be used for payment of mailed affidavits. Those accounts will be handled in the Prepaid Unit of the BMV and may be accessed by phone (614) 752-7598, or by fax (614) 752-7001 to obtain title or registration information. Businesses that have vehicles which do not qualify for the unclaimed motor vehicle affidavit may find these accounts helpful. To set up a prepaid account, contact the Fiscal Section of the Ohio BMV at (614) 752-6674. In the event the unclaimed motor vehicle affidavit submitted cannot be processed, and the BMV 1173 record request is in compliance under R.C. 4501.27, the record request form will be processed in order to assist in notifying the owner and lienholder to settle any outstanding debts. For any vehicles valued at $2,500.00 or more, consider working with the vehicle owner to obtain payment, or a properly assigned title. If this is not possible, legal assistance may be needed. Contact the county title office with questions concerning acceptable documentation. By completing and submitting this form, the custodial business certifies that the following vehicle has been abandoned by the owner and left abandoned for more than fifteen (15) days at the following location.
